Admission Criteria
Magnolia Integrated Health, Inc. have an Admission/Intake Coordinator that performs an intake assessment, which determines if the consumer meets the admission criteria, as well as determining which program is feasible for the consumer.
Please note that the consumer must have Medicaid, Anthem, Aetna, United Healthcare, or other various medical insurance coverage to participate in the programs listed.
Referrals
Individuals participating in programs offered at Magnolia Integrated Health, Inc. may be referred either by another agency or through self referral.
Referrals may be submitted by telephone, faxing a completed referral form to the main office, or by using our secure website to enter and submit referral information.
Contact is attempted with all referrals within 24 hours of receipt of the referral in an effort to schedule an intake assessment.
